Pre-Employment Drug & Alcohol Testing

All applicants for employment will be required to submit to a drug and alcohol test. The Company has adopted cutoff levels set by the Department of Transportation defined in CFR-Part 40. Any applicant who refuses to submit to testing or who receives a result which confirms illegal drug use will be disqualified from further consideration for employment. Refusing to test can include: (a) Refusal to take the test; (b) Failure to report for testing at the time and location instructed to do so; (c) Failure to provide proper identification at the time of testing; (d) Failure to cooperate with collection or testing; or (e) Tampering with, altering of, or substituting urine samples
All applicants, if hired, need to understand that they must report any conviction for drug-related violations occurring on or off Company premises while conducting Company business to the Human Resources Manager. Such a report must be made within five (5) days after conviction.
I have read the Company's abbreviated policy on drugs and alcohol and understand the expectations. I understand that anytime I apply to this Company I will undergo screening for the presence of illegal drugs and breath-alcohol testing for influences above the acceptable limits set by the Department of Transportation.
